国家免费孕前优生检查及风险评估的意义

[摘要]目的 探讨国家免费孕前优生中的风险评估内容和效果。方法 对该县1 025对计划妊娠的夫妇提供了孕前优生服务，对妊娠高危因素进行评估和指导，并对此次检查进行满意度调查。结果 参加该检查的女性有1 025人，其中168例既往有过不良妊娠史；男性发现各项指标异常共有111例，女性有90例；存在高危生育风险的疾病因素包括：男性中有结核感染2例、癫痫1例；女性中妇科相关炎症者38例，甲状腺疾病3例，高血压病35例，糖尿病或空腹血糖异常17例，肥胖47例，高龄41例。家族史高危因素中男性有60例，女性有66例；环境中高危因素男性有118例，女性有49例；随访6个月～2年，失随访86例，未孕541例，已孕398例，其中已分娩176例，足月161例，早产2例，自然流产7例，人工流产6例。满意度抽样问卷调查中，抽取的100例对该检查的总满意度为67.2%，其中认为该服务方便81.2%，检查全面76.1%、沟通完善61.7%，技术优良64.2%，服务态度好70.5%，服务效率高49.7%。结论 推广国家免费孕前优生检查和风险评估能够提高优生工作效果，保护产妇和新生儿安全，对社会和家庭均有积极的意义；应提高孕前优生检查的沟通和服务效率。 [关键词]孕前优生；风险评估；优生服务

[Abstract]Objective To explore the content and effect of risk assessment of free national pre-pregnancy eugenics. Methods 1 025 couples who planned pregnancy in our county were provided with pre-pregnancy eugenics service， their pregnancy risk factors were evaluated， and the satisfaction survey was also conducted. Results Among 1 025 females in the examination， 168 females had a history of adverse pregnancy. 111 males and 90 females had abnormal indicators. Diseases leading to high pregnancy risk included： 2 cases of TB infection and 1 case of epilepsy among males； 38 cases of gynecologic inflammation， 3 cases of thyroid disease， 35 cases of hypertension， 17 cases of diabetes and fasting blood glucose， 47 cases of obesity， and 41 cases of advanced age
